  • The nature of the Li enrichment in the most Li-rich giant star

    Abstract: About 1% of giants have anomalously high Li abundances in their atmospheres, conflicting directly with the prediction of the standard stellar evolution models. This finding makes the production and evolution of Li in the Universe intriguing, not only in the sense of Big Bang nucleosynthesis or the interstellar medium, but also for the evolution of stars. Decades of efforts have been put into explaining why such extreme objects exist, yet the origins of Lirich giants are still being debated. Here we report the discovery of the most Li-rich giant known to date, with a very high Li abundance of 4.51. This rare phenomenon was observed coincidentally with another short-term event: the star is experiencing its luminosity bump on the red giant branch. Such a high Li abundance indicates that the star might be at the very beginning of its Li-rich phase, which provides a great opportunity to investigate the origin and evolution of Li in the Galaxy. A detailed nuclear simulation is presented with up-to-date reaction rates to recreate the Li enrichment process in this star. Our results provide tight constraints on both observational and theoretical points of view, suggesting that low-mass giants can internally produce Li to a very high level through 7Be transportation during the red giant phase.

  • Effects of incremental peritoneal dialysis in patients with urgent-start peritoneal dialysis

    Abstract: Objective: To observe the feasibility and effects of the incremental peritoneal dialysis in patients receiving urgent-start peritoneal dialysis. Methods: This is a single-center retrospective study. ESRD patients who received urgent-start peritoneal dialysis from August 1st2018 through July 31st2021 at the first affiliated hospital of Xi'an Jiaotong University were enrolled to this study. The estimated glomerular filtration rate (eGFR) of all patients was between 4 ~ 6ml/min/1.73m2.According to the difference in the initial dialysis dose, recruited patients were divided into an incremental PD group: initial dialysis dose less than or equal to 6000mlper day, with the dialysis dose being adjusted according to the RRF and dialysis adequacy, and a full-dose PD group: dialysis dose more than or equal to 8000ml per day. Patients were followed-up for 1 year. Clinical biochemical indices, dialysis dose, dialysis adequacy, urine volume, dialysis ultra-filtration volume, residual renal function and peritoneal dialysis-related complications were recorded when peritoneal dialysis was performed at 1,3, and 6-months and 1year. Results: 1. This study examined 169 patients, of which, 111 patients with a mean age of 45.01±12.84 years were in the incremental PD group and 58 patients with a mean age of 43.5±15.62 years were in the full-dose PD group. The demographics, the clinical biochemical indices and residual renal function of both groups before peritoneal dialysis were similar(P>0.05). 2. During follow-up, the dialysis dose in the full-dose PD group exceeded that of the incremental PD group (P<0.05). However, the total Kt/V for both groups exceeded 1.7, and the total Ccr was greater than 50L. At 1 and 6 months, the urine Kt/V in the incremental PD group was higher than that found in the full-dose PD group (P<0.05). 3.During the whole follow-up period, blood pressure control, correction of anemia, and correction of hypocalcemia were also similar for both groups (P>0.05).The correction of hyperphosphatemia in both groups reached the standard.4. During the follow-up period, the residual renal function in both group were similar (P>0.05). The dialysis ultra-filtration volume in the full-dose PD group exceeded that of the incremental PD group, but the differences were not statistically significant (P>0.05).The urine volume of the incremental PD group were significantly higher than that of the full-dose PD group, especially at 1 month and 6 months of dialysis(P<0.05). 5. During the entire follow-up, no patient died in either group. The peritoneal dialysis-related infection, mechanical complications and technical survival rate were similar between groups (P>0.05). Conclusions: The dialysis effect and complications from incremental PD, were similar to full-dose peritoneal dialysis. Incremental PD did not cause a rapid decline of residual renal function in USPD patients. Therefore, USPD patients can be treated by incremental peritoneal dialysis.

  • Measurement of the high energy γ-rays from heavy ion reactions using ˇ Cerenkov detector

    Abstract: The energetic bremsstrahlung photons up to 100 MeV produced in heavy ion collisions can be used as a
    sensitive probe to the short range correlation in atomic nuclei. The energy of the γ-rays can be measured
    by collecting theˇCerenkov light in medium induced by the fast electrons generated in Compton scattering or
    electromagnetic shower of the incident γ ray. Two types of detectors, based on pure water and lead glass as
    the sensitive material respectively, are designed for the above purpose. The γ response and optical photon
    propagation in detectors have been simulated based on the electromagnetic and optical processes in Geant4.
    The inherent energy resolution of 0.022(4) + 0.51(2)/Eγ 1/2 for water and 0.0026(3) + 0.446(3)/Eγ 1/2 for
    lead glass are obtained. The geometry size of lead glass and water are optimized at 30 × 30 × 30 cm3 and
    60 × 60 × 120 cm3 , respectively, for detecting high energy γ-rays at 160 MeV. Hough transform method has
    been applied to reconstruct the direction of the incident γ-rays, giving the ability to distinguish experimentally
    the high-energy γ rays produced in the reactions on the target from the random background cosmic ray muons.

  • Female social entrepreneurship: A research proposal on identity strategies, legitimacy acquisition, and performance impact

    Abstract: Identity, as the starting point for the entrepreneurial behavior and outcomes of female social entrepreneurs, is crucial in addressing the key issues they face - gaining legitimacy and enhancing entrepreneurial performance. However, there is still a lack of research on the relationship between female social entrepreneurs’ identity strategies and legitimacy acquisition, as well as on the underlying mechanisms of how identity integration enhances social entrepreneurial performance. Therefore, this study follows the progressive logic of “identity strategy-legitimacy acquisition- performance impact” to carry out three sub-studies. First, based on the identity theory, we explored the impact of identity strategies on women’s social entrepreneurial decision-making through experimental conjoint analysis. Second, based on the institutional logic perspective, we used fuzzy set qualitative comparative analysis (fsQCA) to explore the configurational impact of identity strategies on legitimacy acquisition. Finally, we constructed a theoretical model based on the identity theory to analyze the impact of female social entrepreneurs’ identity integration on social entrepreneurial performance. This model reveals the mediating role of legitimacy and the moderating role of social bricolage. Our research findings can help guide female social entrepreneurs to develop a clear self-cognition and make informed entrepreneurial decisions to successfully gain legitimacy, thus enhancing social entrepreneurial performance.

  • A High-Mass Young Star-forming Core Escaping from Its Parental Filament

    Abstract: We studied the unique kinematic properties in massive filament G352.63-1.07 at 1000-AU spatial scale with the dense molecular tracers observed with the Atacama Large Millimeter/submillimeter Array (ALMA). We find the central massive core M1 (12 Msun) being separated from the surrounding filament with a velocity difference of v-v_sys=-2 km/s and a transverse separation within 3 arcsec. Meanwhile, as shown in multiple dense-gas tracers, M1 has a spatial extension closely aligned with the main filament and is connected to the filament towards its both ends. M1 thus represents a very beginning state for a massive young star-forming core escaping from the parental filament, within a time scale of ~4000 years. Based on its kinetic energy (3.5x10^44 erg), the core escape is unlikely solely due to the original filament motion or magnetic field, but requires more energetic events such as a rapid intense anisotropic collapse. The released energy also seems to noticeably increase the environmental turbulence. This may help the filament to become stabilized again.

  • Meal rice and soup fish: the southern China factors in the Qi Min Yao Shu

    Abstract: Based on the content of "Qi Min Yao Shu" from chapter one to nine, starting from products and technology, this article tries to reveal the southern China agricultural factors it contained, to explore ways that those factors enter into the north, in order to show the cultural exchanges and agricultural technology dissemination between Southern and Northern China that has been ignored by previous studies. Although the first nine chapter of "QiMinYaoShu" is considered to be the north book reflects agriculture of the Yellow River, the so-called "zhongguo中国" under the rule of the North Wei. But it contains many atypical factors, such as rice, fish, bamboo, orange, water shield, lotus, gravy, water chestnut, wild rice, cabbage, ginger, Yuan(杬),Yue(越) melon, buffalo and soon. Through the ancient natural environment in north China provides some possible existence for these factors, the place name, rice techniques and fish distributions how the involvement of some factors come from the south. So the possible routes, the exchanges both personals and text,that the South factors be carried into the north, are discussed in this article.
     

  • Challenges and Optimization Paths for the Development of Community Health Service in China

    Abstract: In the new era,the health needs of residents are constantly increasing,and how community health services can adapt to the needs of the times and achieve comprehensive transformation urgently needs to be answered. Firstly,the article reviewed the development process of community health service(CHS)in China and found that it is functionally defective. Secondly,the article summarized the challenges faced by the development of CHS in China and pointed out that the current health needs of the population are undergoing structural changes. Finally,the article proposed the development paths of CHS in China in terms of changing the focus of work,improving the sense of value for contracted doctors and the sense of achievement for residents,building a collaborative and complementary CHS network,and establishing a demand-oriented health information platform.

  • A three-level meta-analysis of the relationship between family dysfunction and mental health of children and adolescents

    Abstract: Family dysfunction, characterized by a family's inability to fulfill its roles or the absence of positive characteristics, is a crucial factor influencing the mental health of children and adolescents. However, the nature of this relationship remains a subject of debate. To gain a relatively comprehensive understanding of the link between family dysfunction (both subjective and objective) and mental health (both positive and negative) in children and adolescents, this study employed a three-level meta-analysis. It drawn upon the family systems theory and the two-factor model of mental health as foundational frameworks. We reviewed and screened literature published up to March 1, 2022. Ultimately, 97 studies were included in the analysis, encompassing 173 effect sizes and 130,227 participants.
    The main effect analysis revealed that single-parent families adversely affect the mental health of children and adolescents, exacerbating mental health issues. Other factors such as parental divorce, incarceration, substance abuse, mental illness, and subject family dysfunction also contribute to worsening mental health issues in this demographic. Additionally, the moderating effect analysis indicated that the negative impact of single-parent families is more pronounced in boys. Furthermore, in collectivist cultures, the detrimental effects of parental incarceration on children's and adolescents' mental health are more significant.
    Overall, the findings from this meta-analysis supported the family systems theory. The study highlighted the importance of paying closer attention to the positive aspects of mental health in children and adolescents from dysfunctional families. It also suggested that interventions should be tailored to account for gender and cultural differences.

  • Atypical radio pulsations from magnetar SGR 1935+2154

    Abstract: Magnetars are neutron stars with extremely strong magnetic fields, frequently powering high-energy activity in X-rays. Pulsed radio emission following some X-ray outbursts have been detected, albeit its physical origin is unclear. It has long been speculated that the origin of magnetars' radio signals is different from those from canonical pulsars, although convincing evidence is still lacking. Five months after magnetar SGR 1935+2154's X-ray outburst and its associated Fast Radio Burst (FRB) 20200428, a radio pulsar phase was discovered. Here we report the discovery of X-ray spectral hardening associated with the emergence of periodic radio pulsations from SGR 1935+2154 and a detailed analysis of the properties of the radio pulses. The complex radio pulse morphology, which contains both narrow-band emission and frequency drifts, has not been seen before in other magnetars, but is similar to those of repeating FRBs - even though the luminosities are many orders of magnitude different. The observations suggest that radio emission originates from the outer magnetosphere of the magnetar, and the surface heating due to the bombardment of inward-going particles from the radio emission region is responsible for the observed X-ray spectral hardening.

  • Search for Ronin Pulsars in Globular Clusters Using FAST: Discovery of Two New Slow Pulsars in M15

    Abstract: Globular clusters harbor numerous millisecond pulsars; however, the detection of long-period pulsars within these clusters has been notably scarce. The search for long-period pulsars encounters significant challenges due to pronounced red noise interference, necessitating the crucial step of red noise removal in the data preprocessing. In this study, we use running median filtering to mitigate red noise in multiple globular cluster datasets obtained through observations with the Five-hundred-meter Aperture Spherical radio Telescope (FAST). Additionally, we estimated the minimum detectable flux density of pulsars ($S_{ rm min}$) considering this processing step, resulting in a function depicting how $S_{ rm min}$ varies with different duty cycles and periods. Subsequently, a systematic search for long-period pulsars was conducted on the globular cluster datasets after red noise elimination. Ultimately, two isolated long-period pulsars were discovered in the M15 globular cluster, with periods of approximately 1.928451 seconds and 3.960716 seconds, both exhibiting remarkably low pulse duty cycles of around 1 %. Using archived data, we obtained timing solutions for these pulsars. Based on the timing results, their positions are found to be close to the center of the M15 cluster. On the $P- dot{P}$ diagram, they both lie below the spin-up line, suggesting that their recycling process was likely interrupted, leading them to become isolated pulsars. Moreover, in our current search, these very faint long-period pulsars are exclusively identified in M15, and one possible reason for this could be the relatively close proximity and extremely high stellar formation rate of M15. As observational data accumulate and search algorithms undergo iterative enhancements, the prospect of discovering additional long-period pulsars within globular clusters, such as M15, becomes increasingly promising.

  • Milestone progress of the HEPS booster commissioning

    Abstract: The high-energy photon source (HEPS) is the first fourth-generation synchrotron light source facility in China. The HEPS injector consists of a linear accelerator (Linac) and a full energy booster. The booster captures the electron beam from the Linac and increases its energy to the value required for the storage ring. The full-energy beam could be injected to the storage ring directly or after “high-energy accumulation.” On November 17, 2023, the key booster parameters successfully reached their corresponding target values. These milestone results were achieved based on numerous contributions, including nearly a decade of physical design, years of equipment development and installation, and months of beam commissioning. As measured at the extraction energy of 6 GeV, the averaged beam current and emittance reached 8.57 mA with 5 bunches and 30.37 nm.rad with a single-bunch charge of 5.58 nC, compared with the corresponding target values of 6.6 mA and 35 nm.rad, respectively. This paper presents the physical design, equipment development, installation, and commissioning process of the HEPS booster.

  • α decay in extreme laser fields within a deformed Gamow-like model

    Abstract: In this study, the effect of extreme laser fields on the α decay process of ground-state even–even nuclei was investigated. Using the deformed Gamow-like model, we found that state-of-the-art lasers can cause a slight change in the α decay penetration probability of most nuclei. In addition, we studied the correlation between the rate of change of the α decay penetration probability and angle between the directions of the laser electric field and α particle emission for different nuclei. Based on this correlation, the average effect of extreme laser fields on the half-life of many nuclei with arbitrary α particle emission angles was calculated. The calculations show that the laser suppression and promotion effects on the α decay penetration probability of the nuclei population with completely random α particle-emission directions are not completely canceled. The remainder led to a change in the average penetration probability of the nuclei. Furthermore, the possibility of achieving a higher average rate of change by altering the spatial shape of the laser is explored. We conclude that circularly polarized lasers may be helpful in future experiments to achieve a more significant average rate of change of the α decay half-life of the nuclei population.

  • The Development of Quality Indicator System for the Home Care Services for the Disabled Elderly in Beijing

    Abstract: Background With the deepening of aging in China,there is an increasing demand for home care services for disabled elderly individuals. The evaluation of service quality provides an assurance for the implementation of high#2;quality services. Objective To develop a quality indicator system for the delivery of home care services to the disabled elderly population in Beijing based on the Delphi method,so as to offer objective criteria for evaluating the quality of home care services provided by community health service institutions. Methods Based on literature research and qualitative study,a preliminary pool of indicator system entries for the indicator system was formed,and an expert correspondence form was designed. A total of 21 community nursing,general practice,and management research personnel experts with rich experience in the field of home medical services and elderly comprehensive assessment in Beijing were selected,and an expert correspondence form was sent to them by email from June to October 2021 to conduct three rounds of Delphi expert correspondence. The experts' personal information and authority were recorded and analyzed,to the establish a quality indicator system for home care services for disabled elderly individuals in Beijing. Results All three rounds of expert correspondence form were returned and valid,with a 100% positive coefficient of experts from the 21 selected experts who had a mean expert authority score of 0.927 (ranging from 0.700 to 1.000),indicating an acceptable level of expertise. The first,second,and third rounds of expert correspondence yielded importance and feasibility coordination coefficients of 0.170 and 0.140(P<0.001),0.147 and 0.175 (P<0.001),0.231 and 0.208 (P<0.001),respectively,indicating a high level of consensus among the experts and the reliability of the correspondence results. The resulting quality indicator system for home care services for disabled elderly individuals included three primary indicators of service conditions,service processes,and service outcomes,as well as nine secondary indicators and 34 tertiary indicators. Conclusion  The indicator system constructed in this study is appropriate for evaluating the implementation quality of single visit medical service,and providing a reference for community health service institutions to formulate regulations and work programs.

  • Discussion on the Current Situation and Development Path of Primary TCM Health Service System in China

    Abstract: Background  Primary traditional Chinese medicine(TCM) health service has been attracting continuously improving attention. However,primary TCM health service system still has problems,which have constrained the development of TCM in China. Objective  To analyze the current situation of primary TCM health service system in China and explore the strategies to continuously improve the capacity of primary TCM health service. Methods  In December 2022,data were retrieved from the China Health Statistics Yearbook,National Statistics of Traditional Chinese Medicine,the official websites of the National Bureau of Disease Control and Prevention and the National Center for Cardiovascular Diseases. The index system was constructed from the three levels of health input,service output and health needs,and the combination of rank sum ratio method and the WHO health system performance module framework was used to comprehensively evaluate the current situation of primary TCM health service. Results  In terms of health input,from 2017 to 2021,the number of primary healthcare institutions providing TCM health services except for township health centers has increased,and the number of TCM clinical departments beds in community health service centers(stations),township health centers,and outpatient clinics has shown an increasing trend,but the proportion of TCM clinical departments beds in outpatient clinics to the total number of outpatient departments has decreased;the proportion of TCM practicing(assistant) physicians in community health service centers(stations),township health centers and clinics has increased significantly,while the proportion of TCM practicing(assistant) physicians in village clinics remained stable,and the proportion of TCM practicing(assistant) physicians in outpatient clinics decreased by years;in the past five years,the absolute value of financial input for primary healthcare institutions has steadily increased,while the financial allocation for TCM organizations has fluctuatingly decreased. In terms of service output,from 2017 to 2021,the proportion of TCM consultations to the total consultations in various institutions has steadily increased according to a certain increase(except for TCM outpatient clinics),while the proportion of TCM consultations in village clinics alone has always remained above 30.0%;in addition to township health centers,the per capita burden of consultations of TCM practicing(assistant)physicians in other primary healthcare institutions was generally on a downward trend. The comprehensive evaluation found that 2017,2018 and 2021 were in a suitable state of resource allocation,while 2019 and 2020 were in an unbalanced state of resource allocation. Conclusion  At present,the construction of primary TCM health service system in China has been effectively implemented,but there are still some problems in the process of orderly promotion. Problems such as insufficient resources for primary TCM,unbalanced service and utilization,insufficient advantages of TCM,imperfect personnel training mechanism and low financial input have constrained the development TCM. In order to promote the continuous improvement of the health service capacity of primary TCM health service institutions,it is necessary to consolidate the functions of government,further promote TCM culture,establish incentive mechanism to optimize the training mode of talents,strengthen the close integration of TCM health services with health insurance policies,and build a TCM medical association supported by "information integration".

  • Sinicization Based on Community Health Intensity Rating Scale and Its Reliability and Validity Test in Elderly Patients

    Abstract: Background  In the context of the national efforts to promote the development of home care services for the elderly,chronic diseases and disabled patients,the adoption of professional assessment tools that can effectively assess the health of patients in the home environment and meet the needs of care services is an important guarantee to accurately match the needs of patients for home medical care. Objective  To sinicize the Community Health Intensity Rating Scale(CHIRS)and evaluate its reliability and validity in the elderly patients with chronic diseases in the community. Methods  After obtaining authorization from the original author,the translation,back translation and cultural adaptation process of the Brislin model were followed to form the Chinese version of CHIRS. From March to June 2021,a convenience sampling method was used to select elderly patients managed by a community health service center(station)in Chengdu as the research subjects. Expert consultation was used to evaluate the content validity of the scale; the internal consistency coefficient was used to test the reliability of the scale and to verify its practicality. Results  A total of 244 patients were investigated and completed the household questionnaire,with a valid recovery rate of 100%. The expert consultation results showed that the S-CVI/ave of the Chinese version of CHIRS was 0.98,and the I-CVI ranged from 0.71 to 1.00; the overall Cronbach'α coefficient was 0.884,and the Cronbach'α coefficients for the four dimensions ranged from 0.593 to 0.787,the predictive validity results showed that the Spearman correlation coefficients of CHIRS result with self-rated health status and demand for home care services were -0.611(P<0.001)and 0.584(P<0.001). Conclusion  After sinicization and localization modification,the Chinese version of CHIRS has better reliability and reference value,which can be used to evaluate the health status and demand for home care services of elderly patients with chronic diseases in the community.

  • Development and Application of Document Delivery Robot Based on the Selenium Framework

    Abstract:   Purpose/significance The author is responsible for document delivery work on the Jiangsu Engineering Technology Literature Information Center platform where mechanical repetitive operations account for a large proportion, seriously encroaching on work time. Moreover, the literature requirement is 24 hours, and as a duty librarian, it can only be transmitted within a limited time, making it difficult to ensure the timeliness of transmission. So, the author attempts to develop a document delivery robot to automate the completion of document delivery work. Method/process The paper takes the Jiangsu Engineering Technology Literature Information Center platform as an example, based on the Selenium framework, designs and develops a document delivery robot, which realizes the entire process of logging in, checking in, obtaining orders, searching, downloading, uploading, completing literature transfer, and then continuing to wait for dispatching. Result/conclusion After a month of operation analysis, it was found that although the document delivery robot cannot completely replace manual delivery, it can effectively solve the high proportion of mechanical repetitive work in the literature delivery process, improve the delivery efficiency, and expand the service time, allowing the delivery librarians to have more energy to solve difficult to find literature such as special literature. Finally, document delivery robots also have reference significance for the field of document delivery and other related scenarios.
     

  • Relativistic ab initio studies for nuclear matter and neutron star matter

    Abstract:相对论 Brueckner-Hartree-Fock (RBHF) 理论是相对论框架下重要的第一性原理方法,仅包含两体力即可以满意描述核物质的饱和性质。在完备的 Dirac 空间中自洽求解核物质 RBHF 方程,唯一确定了单粒子势的标量和矢量分量,避免了已有工作中由于忽略负能态所导致的不确定性,解决了 40 多年来 RBHF 计算中不能唯一确定单粒子势的问题。文章简要回顾 RBHF 理论的发展历史,阐述包含负能态做 RBHF 计算的必要性,介绍利用完备 Dirac 空间的 RBHF 理论,研究核物质性质和中子星物质性质的最新进展,包括有效质量、纯中子物质的每核子结合能、对称和纯中子物质的压强、中子星物质的粒子分数和状态方程、中子星的质量半径关系和潮汐形变等。文章还展望了完备 Dirac 空间的RBHF 理论在确定密度泛函理论参数、描述核子原子核弹性散射和研究中子星内部强子夸克相变等方面的可能应用。

  • Conceptual Design of a 714-MHz RFQ for Compact Proton Injectors and Development of a New Tuning Algorithm on its Aluminium Prototype

    Abstract: Radio frequency quadrupoles (RFQs), which are crucial components of proton injectors, significantly affect the performance of proton accelerator facilities. An RFQ with a high frequency of 714 MHz dedicated to compact proton injectors for medical applications is designed in this study. The RFQ is designed to accelerate proton beams from 50 keV to 4 MeV within a short length of 2 m and can be matched closely with the downstream drift tube linac to capture more particles through a preliminary optimization. To develop an advanced RFQ, challenging techniques, including fabrication and tuning method, must be evaluated and verified using a prototype. An aluminium prototype is derived from the conceptual design of the RFQ and then redesigned to confirm the radio frequency performance, fabrication procedure, and feasibility of the tuning algorithm. Eventually, a new tuning algorithm based on the response matrix and least-squares method is developed, which yields favorable results based on the prototype, i.e., the errors of the dipole and quadrupole components reduced to a low level after several tuning iterations. Benefiting from the conceptual design and techniques obtained from the prototype, the formal mechanical design of the 2-m RFQ is ready for the next manufacturing step.
     

  • Reliability and Validity of the Chinese Version of the Modified COVID-19 Yorkshire Rehabilitation Scale

    Abstract: Background  The modified COVID-19 Yorkshire Rehabilitation Scale(C19-YRSm) has been shown to be useful to assess and monitor patients in post-COVID-19 syndrome in developed countries. However,its reliability and validity in the Chinese population remains unclear. Objective  To translate the C19-YRSm into a Chinese version(C19-YRSm-C),and test the reliability and validity of the Chinese version. Methods  After obtaining the approval of the translation and use authority of the C19-YRSm from MANOJ SIVAN's team at the University of Leeds,United Kingdom,the research team translated the C19-YRSm and formed the C19-YRSm-C after translation,back-translation,and cross-cultural adaptation,according to the Translation and Cross-cultural Adaptation Procedure. From May 1 to 7,2023,a simple random sampling method was used to select post COVID-19 patients from the "Wen Juan Xing" platform sample pool as the study subjects. The General Information Questionnaire and the C19-YRSm-C were used to survey them. 13 experts were invited to evaluate the content validity of the scale. The C19-YRSm-C was subjected to item analysis,reliability and validity analysis. Results  A total of 512 questionnaires were distributed and 370 valid questionnaires were recovered,with a valid recovery rate of 72.3%. The results of item analysis showed that the CR values of the items ranged from 6.589 to 22.725(P<0.001),and the item-total correlation coefficients of the scale ranged from 0.547 to 0.806 (P<0.001). For reliability,the Cronbach's α coefficient,Guttman Split half coefficient and test-retest reliability of the C19-YRSm-C was 0.881,0.837 and 0.816,respectively. For content validity,the item-level content validity index(I-CVI) of the C19-YRSm-C ranged from 0.692 to 1.000,and the scale-level validity index(S-CVI) was 0.914. The results of confirmatory factor analysis(CFA) showed that the fitting indicators of the initial model were undesirable. After the establishment of a covariance correlation between error variables e12 and e13 according to the indication prompts,the fitting indicators of the modified model were within the acceptable range except for the adjusted goodness of-fit index (AGFI). The results of discriminant validity analysis showed that the differences in scores for the 4 subscales of the C19-YRSm-C among post-COVID-19 patients with different chronic disease history and cumulative number of COVID-19 showed statistically significant differences(P<0.05). The differences were statistically significant when comparing the scores of the 3 subscales in addition to other symptoms before and after COVID-19 infection(P<0.05). Conclusion  The C19-YRSm-C has good reliability and validity,with good applicability in Chinese population,which can be used to assess and monitor the rehabilitation status of post COVID-19 patients in China.

  • Clinical Characteristics of Patients with Idiopathic Inflammatory Myopathy and Risk Factors for the Development of Interstitial Lung Disease

    Abstract: Background  Idiopathic inflammatory myopathy(IIM) is a group of connective tissue diseases characterized by muscle inflammation and muscle weakness,lung involvement is an important factor affecting the prognosis of patients. IIM can be classified into different clinical subtypes based on myositis-specific antibodies(MSAs),there are significant differences in clinical manifestations,organ involvement,prognosis and the risk of interstitial lung disease among different clinical subtypes. Objective  To explore the characteristics of IIM and its different clinical subtypes,the risk factors for the development of interstitial lung disease. Methods  The clinical data of patients hospitalizedin department of rheumatology and immunology of the First Affiliated Hospital of Kunming Medical University and diagnosed with IIM from April 2018 to February 2021 were collected. The included patients were divided into four clinical subtypes based on MSAs,including anti-MDA5-positive dermatomyositis (DM),anti-MDA5-negative DM,immune-mediated necrotizing myositis (IMNM) and anti synthetase syndrome (ASS) subtypes. The general data,clinical manifestations,laboratory examination results were compared among different clinical subtypes. Multivariate Logistic regression model was established to explore the risk factors for ILD in patients with IIM. Results  The 150 patients with IIM were divided into 4 clinical subtypes,including 30 patients with anti MDA5-positive DM subtype(20%),58 patients with anti-MDA5-negative DM subtype(38.7%),14 patients with IMNM subtype(9.3%),and 48 patients with ASS subtype(32.0%). There were significant differences in the incidence of muscle weakness,myalgia,ILD,heliotrope rash,shawl sign,Gottron papules or Gottron sign,arthralgia,periungual red spot and dysphagia among the four clinical subtypes(P<0.05). The incidence of ILD in patients with anti-MDA5-positive DM and ASS subtypes was higher than patients with anti-MDA5-negative DM and IMNM subtypes,respectively(P<0.05);The incidences of heliotrope rash and shawl sign in patients with anti-MDA5-positive DM and anti-MDA5-negative DM subtypes were higher than patients with IMNM and ASS subtypes(P<0.05);The incidences of arthralgia in patients with anti-MDA5-positive DM subtype was higher than patients with anti-MDA5-negative DM subtype(P<0.05). There were significant differences in the levels of WBC,ALT,AST,serum creatinine,LDH,CK,C4,ferritin,T cell,CD8+ T cell,nature kill (NK) cell and incidence of ILD among patients with different subtypes(P<0.05). Multivariate Logistic regression analysis showed that anti MDA5 antibody positive,anti-synthetase antibody positive,lung infection,ferritin>403.2 μg/L,IgG>14.15 g/L,LDH>359.5 IU/L were all risk factors for ILD in IIM. Conclusion  The clinical manifestations of patients with different clinical subtypes differ significantly. DM patients with anti-MDA5 antibody positive are more likely to develop rash,arthralgia,ILD and leukopenia. MDA5 antibody positive,anti-synthetase antibody positive,lung infection and elevated ferritin,LDH and IgG levels are the risk factors for IIM with ILD.
