  • Influence of Policy Factors on Healthcare-seeking Behaviors in Patients with Schizophrenia

Subjects: Medicine, Pharmacy >> Clinical Medicine submitted time 2023-04-28 Cooperative journals: 《中国全科医学》

Abstract: Background In China, schizophrenia patients have a high demand for basic medical care, but irrational healthcare-seeking behaviors among them may cause a waste of health resources. So relevant policies have been promulgated to improve the ensuring of healthcare and reasonably guide schizophrenia patients to seek medical care. The current research on policy factors and healthcare-seeking behaviors has rarely addressed the association between mental health policies and healthcare-seeking behaviors of patients with schizophrenia. Objective To analyze the influence of policy factors on healthcare#2;seeking behaviors in schizophrenia patients,to understand the status and outcome of health service utilization in this group under the guidance of basic medical insurance policies and mental health policies, providing empirical evidence for policy improvement. Methods In November 2019,we obtained a sample of 2 314 schizophrenic individuals with valid key data by matching the personal data in the three-level psychiatric prevention and treatment network system in a district of Chongqing in 2018,with information related to personal health service utilization in the district obtained from the medical insurance information platform of Chongqing Human Resources and Social Security Bureau. Multivariate Logistic regression analysis was conducted with healthcare#2;seeking behaviors and the medical institution chosen for treatment as explained variables, participation in basic medical insurance(including basic medical insurance for rural and urban non-working residents or basic medical insurance for urban employees, and reimbursement for medical cost due to special outpatient diseases) and mental health policy coverage(involving community rehabilitation services, guardianship subsidy, subsidy from the local disability federation, subsidy from the 686 program, and free second-generation antipsychotics, and the mental disability certificate) as key explanatory variables, and patients'demographic characteristics as control variables. Results Of the participants,1 915(82.76%) had medical visits, including 1 482 seeking outpatient treatment, and 795 seeking inpatient treatment. Among those seeking outpatient treatment, the prevalence of choosing primary, secondary and tertiary care institutions was 17.01%(252/1 482),72.87%(1 080/1 482),and 10.12%(150/1 482),respectively. And the prevalence of choosing primary, secondary and tertiary care institutions for inpatient treatment was 4.78%(38/795),65.16%(518/795),and 30.06%(239/795),respectively. Logistic regression analysis showed that the type of basic medical insurance, reimbursement for medical cost due to special outpatient diseases, receiving community rehabilitation services, enrolment in the 686 program and involvement in the free second-generation antipsychotics program were factors associated with healthcare-seeking behaviors(P<0.05). The type of basic medical insurance, reimbursement for medical cost due to special outpatient diseases, receiving community rehabilitation services, enrolment in the free second-generation antipsychotics program, and the level of mental disability were factors associated with choosing outpatient settings for treatment(P<0.05). The type of basic medical insurance and the level of mental disability were associated with choosing inpatient settings for treatment(P<0.05). Conclusion Policy factors played a positive role in guiding schizophrenia patients to seek medical treatment, but some problems were also revealed, such as low patient participation and insufficient coverage pathways. In view of this, efforts should be made as soon as possible to optimize mental health policies and the social support and assistance system, increase patient participation of relevant programs via strengthening the publicity of relevant policies, actively guide home-based rehabilitation activities, further promote community-based delivery of mental health services, and continuously improve the mental health security system.

Subjects: Medicine, Pharmacy >> Clinical Medicine submitted time 2023-04-28 Cooperative journals: 《中国全科医学》

Abstract: Background Breast cancer is the most prevalent malignancy in the world. Endocrine therapy reduces the level of estrogen in vivo, thus affecting the blood lipid level, which reduces the quality of life and the treatment compliance of patients. Objective To analyze the clinical characteristics and traditional Chinese medicine(TCM)prescription of hyperlipidemia in estrogen receptor(ER)positive breast cancer patients during endocrine therapy in real world. MethodsBased on method of retrospective study,a total of 238 patients with ER positive breast cancer who received endocrine therapy in Shandong Province Hospital of Traditional Chinese Medicine from 2012 to March 2022 were selected as the study subjects. The data including age, triglyceride, total cholesterol, low density lipoprotein, endocrine therapy drugs,TCM prescriptions of the patients were exported through the search platform of scientific research big data to establish a clinical data table of the patients. ER positive breast cancer patients with normal baseline blood lipid levels were divided into the normal group and dyslipidemia group according to the blood lipid levels during endocrine therapy, and hyperlipidemia was classified into hypercholesterolemia, hypertriglyceridemia and mixed hyperlipidemia. The exported prescriptions were analyzed for frequency, four properties, five flavors, channel tropism and medication regularity by using the 〝prescription analysis",an auxiliary platform for TCM inheritance,to obtain new prescriptions. Results Among 238 ER positive breast cancer patients,97 patients(41.00%)had normal baseline blood lipid levels,of whom 42 patients developed dyslipidemia during standardized endocrine therapy. Among the patients with dyslipidemia,77 person-time(37.6%)occurred in the age group of 51 to 60 years as the highest number;86 person-time with dyslipidemia received exemestane treatment, accounting for the highest proportion of 41.9%. Among the 42 patients with dyslipidemia(205 person-time in total),hyperlipidemia occurred in 99 person-time,and hypercholesterolemia occurred in 49 person-time(54.13%). There was no statistically significance difference in the age of patients with three types of hyperlipidemia(P>0.05). There were statistically significance differences in the proportion of endocrine therapy types among patients with three types of hyperlipidemia(P<0.05). There were 189 kinds of TCM prescriptions for hyperlipidemia in ER positive breast cancer patients during endocrine treatment, licorice was the most frequently used medicine, the highest frequency of use was tonifying qi drug. The channels of TCM collected by the big data platform were mainly distributed in the spleen, lung and liver channels. The properties of TCM were mainly cold, flat and warm, and the flavors were mainly sweet, bitter and pungent. The combination of 〝Astragalus and Licorice" had the highest frequency. Six core drug combinations and three new prescriptions were obtained through complex system entropy clustering analysis. Conclusion Based on analysis of clinical characteristics and prescription medication, the position of hyperlipidemia in ER positive breast cancer patients during endocrine therapy is in the spleen, with pathogenesis of dysfunction of spleen in transportation and phlegm turbidity stagnation. The medication are maily used for invigorating spleen to remove dampness, tonifying qi and regulating stomach.

Abstract: This paper studies the disyllabic patterns of four disyllabic dialects in Lan-Yin Mandarin, describes and compares the monosyllabic patterns and disyllabic patterns of the four disyllabic dialects. Then, from an acoustic perspective, a comparative analysis was conducted on the differences in duration, pitch, and intensity between two tone tones and two syllable words within four two tone dialects. The results showed that the duration of two tone tones was significantly longer than that of two syllable words, and there were corresponding differences in pitch and intensity curves. The intensity of two tone tones had a clear valley. Finally, from the perspective of language contact, this paper analyzes the causes of the double tone characteristics of Lan-Yin Mandarin's two tone dialect, and points out the significance of the comparative study of the double syllable words and double tone.

  • Research on Chinese Phonetic Awareness: Taking Cantonese Dialect as an Example

Subjects: Linguistics and Applied Linguistics >> Linguistics and Applied Linguistics submitted time 2023-04-28

Abstract: This study explores the ability of Chinese adults to detect and manipulate different speech units through speech awareness testing, and discusses the impact of rhyming and basic speech perception units in Chinese populations, as well as the knowledge of Chinese pinyin, on the establishment of speech awareness in Chinese populations. The experiment examined the performance of two groups of adult participants (each consisting of 15 people) with/without Pinyin ability in the local dialect of Duanzhou, Zhaoqing, in seven phonological awareness tests (syllable awareness, vowel detection, tone detection, initial detection, vowel substitution, tone substitution, and initial deletion). The test results showed that both groups of participants had complete syllable awareness; The Pinyin group is proficient in detecting and manipulating tonal units, while non Pinyin groups find it difficult to operate longitudinal tones alone, but have a certain level of detection ability. Among them, detecting vowels is the best, followed by tones, and vowels are the worst; The ability to generate rhyming syllables with a given stimulus without the need to extract vowels from the syllables; Pinyin groups can cut syllables at the sound vowel boundary, while non pinyin groups cannot; The acquisition of Pinyin will change the basic perception mode of Chinese phonetics.

  • On Phonetic and Phonetic Systems: Taking Chinese as an Example

Subjects: Linguistics and Applied Linguistics >> Linguistics and Applied Linguistics submitted time 2023-04-28

Abstract: With the progress of phonetics technology and research methods, phonemics has gradually developed from the methodology of structuralism to the methodology of cognitive science, and gradually established the "cognitive phonemics". From the perspective of cognitive phonemics, phonetic awareness is the first step in establishing phonemes. Phonemic awareness is usually defined as the phonetic units that can be perceived by native speakers without discriminating meanings. Phonetic awareness can be divided into natural phonetic awareness and unnatural phonetic awareness. Based on these basic concepts, this paper discusses the phonemic system of Chinese in different historical periods: 1) Syllabic phoneme system "Zhiyin"; 2) phonemic system of initials and finals "Fanqie"; 3) Quasi-phonemic system of initials, rhymes and tones "Guang Yun"; 4) Quasi-phonemic system of initials, rhymes and tones "Zhuyin Fuhao"; 5) Phonemic system of segments "Hanyu Pinyin". The study found that the phonemic system of Chinese gradually changed from the phonemic system based on natural phonetic awareness to the phonemic system based on unnatural phonetic awareness. From the point of view of phonetic units, it also gradually changes from syllables to segments. Based on these studies and findings, this paper gives a clear definition of "phonemic system" and "phonetic notation system", and expounds the difference between them. According to the definitions of "phonemic system" and "phonetic notation system", it is found that most of the field investigations and studies on phonemic construction of Chinese-Tibetan languages have established "phonetic notation system" rather than "phonemic system".

  • The Modeling and Optimization of Building the Multi-dam System on Zambezi River

Subjects: Mathematics >> Mathematics (General) Subjects: Hydraulic Engineering >> Basic Disciplines of Hydraulic Engineering submitted time 2023-04-28

Abstract: In this paper, we mainly provide a proper maintenance plan for the Kariba Dam in Africa which falls into disrepair and is facing to collapse. Firstly, we make a threshold analysis of the three options about their costs which include people’s moving, old dam’s removing, new dams’ building, later repairing, ecological destruction and their incomes which include generation energy, avoiding of flood disasters’ loss, providing employment, tourism resources and ecological protection. Then we get the specific relationship between benefits and years with some collected data. Both of the results show that the third option is the best choice from the economic view. And the result is completely as same as the conclusion we get after studying deeply on Option 3. Secondly, we regard water management capabilities as the safety coefficient of dams. We select 30 seed points along the riverbank for preparing the establishment of dams. With flow-between-riverway model, Manning equations, large Cauchy distribute function we get the scores of the seed points. We give an advice that the number of dams should be more and the positions of dams should be well-distributed. Then, we build an assessment model by analytic hierarchy process. We select three factors among all the factors, safety, economy and population. After testing the consistency, we get the weights of each factor: 0.6442, 0.2705, 0.0852. Then we value the factors and get an optimal scheme during the assessment with 0-1 integer programming: the number of dams is 17 and the longitude and latitude of them are shown in Table 17. The sensitivity of the result is tested as well. We also provide some strategies for the managers of ZRA to use. We suggest that they should use the dams normally in general. With the Dam-break model, we find 13 points among 17 points which are shown in Table 20. The dams at the 13 points need to be closed when there is a flood and it is just the opposite when the drought happens. For the extreme water flow, we assume an ideal water flow at first. The extreme water flow has to be adjusted to satisfy the ideal one. As for the restrictions in extreme conditions, the biggest impact happens at the 8th point among the 17 points. If the duration of maximum flow is t0, the drainage time t to make the water flow return to the normal level equals to 4.95t0.

  • Correlation between Estimated Glucose Disposal Rate and Metabolism-associated Fatty Liver Disease in Type 2 Diabetes

Subjects: Medicine, Pharmacy >> Clinical Medicine submitted time 2023-04-27 Cooperative journals: 《中国全科医学》

Abstract: Background Metabolism-associated fatty liver disease(MAFLD)is considered as a major cause of increased morbidity and mortality from liver disease, type 2 diabetes mellitus(T2DM)is a driving factor in the progression of MAFLD. Estimated glucose processing rate(eGDR)is a simple evaluation indicator of insulin resistance in patients with T2DM,while its relationship with MAFLD has been rarely studied. Objective To investigate the correlation between eGDR and MAFLD in type 2 Diabetes and its predictive value. Methods A total of 1 434 patients with T2DM who were hospitalized in Hebei Provincial People's Hospital from 2019-01-01 to 2019-12-31 were selected as the study subjects. baseline data of the patients was collected, with their venous blood from the elbow collected for laboratory examination, and liver condition examined by abdominal ultrasound. According to the results of abdominal ultrasonography,T2DM patients were divided into patients with MAFLD(n=734)and patients without MAFLD(n=700). The subjects were divided into T1 group(eGDR ≤ 5.09,n=477),T2 group(5.097.11,n=479)according to the eGDR tertiles. Spearman rank correlation analysis was used to explore the correlation between eGDR and baseline data. Univariate and multivariate Logistic regression analysis was used to explore the influencing factors of combined MAFLD. The multiplicative interactions of eGDR with gender, age, hypertension, glycated hemoglobin A1c(HbA1c),BMI and glutamyl transferase(GGT)were evaluated. Receiver operating characteristic (ROC)curve was plotted to evaluate the predictive value of combined prediction model,FBG and HbA1c for MAFLD in T2DM and areas under curve(AUC)were calculated and compared by Delong test. Results Age,disease course,HDL-C and eGDR of patients with MAFLD were lower than those without MAFLD. BMI,alcohol consumption, proportions of hypertension and smoking, fasting blood glucose(FBG),uric acid(UA),total cholesterol(TC),TG,low density lipoprotein cholesterol(LDL-C),alanine aminotransferase(ALT),aspartate aminotransferase(AST),GGT and ALB were higher than those without MAFLD(P<0.05). There were statistically significant differences in age, combined MAFLD,BMI and hypertension,FBG,HbA1c,UA,creatinine(Scr),TG,HDL-C,GGT and ALB among T1,T2 and T3 groups(P<0.05). Among them, the proportion of hypertension,age,BMI,FBG,HbA1c,Scr and TG in T3 group were lower than those in T1 and T2 groups,GGT was lower than that in T1 group,HDL-C and ALB were higher than those in T1 and T2 groups, the proportion of hypertension,BMI,FBG,HbA1c and TG in T2 group were lower than those in T1 group(P<0.05). eGDR was negatively correlated with age,FBG and ALB in patients with MAFLD(P<0.05). In patients without MAFLD,eGDR was negatively correlated with age, disease course,FBG,Scr,TG and ALB(P<0.05),and positively correlated with HDL-C,AST and GGT(P<0.05). Multivariate Logistic regression analysis showed that eGDR〔OR=0.814,95%CI(0.772,0.858),P<0.001〕was an influencing factor of MAFLD. Compared with T3 group, the risk of combined MAFLD was higher in T1 and T3 groups(P<0.05). BMI(Pinteraction<0.001),GGT(Pinteraction=0.033),hypertension(Pinteraction<0.001)had interaction with eGDR. The AUC of the combined prediction model was 0.743,which was greater than FBG(Z=3.227,P=0.001)and HbA1c(Z=1.877,P=0.061). Conclusions The level of eGDR in T2DM patients with MAFLD is significantly lower than that in patients without MAFLD,and patients with low eGDR level have a higher risk of MAFLD. eGDR is a risk factor for MAFLD in T2MD patients. The combined prediction model of eGDR can be used as a predictor to evaluate the risk of MAFLD in T2MD patients.

Subjects: Medicine, Pharmacy >> Clinical Medicine submitted time 2023-04-27 Cooperative journals: 《中国全科医学》

Abstract: Background Glycemic control is usually poor in patients with endogenous-insulin-deficient diabetes, while low-carbon diets(LCD) are beneficial for glycemic control among patients with diabetes. However, few studies on LCD in patients with endogenous-insulin-deficient diabetes have been reported. Objective To investigate the efficacy and safety of LCD for patients with endogenous-insulin-deficient diabetes. Methods A total of 80 patients with endogenous-insulin-deficient diabetes(fasting C-peptide ≤ 0.5 ng/mL) who were hospitalized in Shanghai Eighth People's Hospital from August 2018 to August 2022 were selected as research subjects and divided into the control diet(CD) group with 55 cases and LCD group with 25 cases. The CD group and LCD group were matched by propensity score matching(PSM) in a 1∶1 ratio based on age and diabetes duration as matching indicators and caliper value as 0.3. The two groups of patients were compared in terms of glycemic control on the first day of hospitalization and at discharge, including average blood glucose, coefficient of variation(CV),percentages of time in range (TIR),time below range (TBR) and time above range(TAR);use of antidiabetic drugs, including the numbers and dosages of daily insulin injection and the types of oral antidiabetic agents;incidence of adverse events, including proportion of individuals who developed diabetic ketoacidosis(DKA) or hypoglycemia. Results After PSM,22 patients were included in the CD group and LCD group, respectively. The percentages of CV(25.92% vs. 33.79%) and TAR(20.19% vs. 30.43%) were lower in the LCD group than in the CD group(both P<0.05),the percentage of TIR(77.79% vs. 67.05%) was higher in the LCD group than in the CD group(P<0.05). The numbers(3.0 times vs. 1.5 times) and dosages(17.4 U/day vs 29.3 U/day) of daily insulin injection and the types of oral antidiabetic agents(2 types:0% vs. 18.2%;1 type:31.8% vs. 54.5%;0 type:68.2% vs. 27.3%) in the LCD group were significantly lower than those in the CD group (P<0.05). There was no significant difference in the incidence of adverse events(18.2% vs 22.7%) between the two groups during hospitalization(P=1.000). Conclusion Compared to general diet,LCD is beneficial in reducing glucose variability, prolonging TIR,reducing the use of antidiabetic agents without increasing the risk of DKA or hypoglycemia for patients with endogenous-insulin-deficient diabetes.

  • The Association between Transgressor’s Remorse and Victim’s Forgiveness among Young Children: The Activation Effect of Bystanders

Subjects: Psychology >> Developmental Psychology submitted time 2023-04-26

Abstract: Humans are extremely social beings, and we attempt to repair our ruptured relationships when transgressions occur that damage interpersonal cooperation. The expression of guilt andremorse by the transgressor and the forgiveness by the victim are both vital to the repair process. To some extent, transgressors’ remorse is the most prominent elicitor of victims’ forgiveness. Previous studies have demonstrated that forgiveness emerges as early as 5 years old and that young children are capable of forgiving a remorseful transgressor even in the absence of anexplicit apology. Given the emphasis on relationship harmony among Chinese people in a collectivistic culture, parenting and socialization might help children understand peers’ remorse intentions and forgive them at a much earlier age. Furthermore, the high need for personal reputation and social image in peer interactions, which is called “face” (mianzi) by the Chinese, might lead to individuals’ forgiveness decisions being influenced by bystanders who witness or participate in group interactions. Therefore, this study aimed to investigate whether 4- or 5-year- olds could have the capacity to infer an apology from displays of transgressors’ remorse in order to reveal the influence of remorse on forgiveness and further explore the activation effect of different types of bystanders among the association between remorse and forgiveness. Study 1 was a one-factor (expression of remorse: remorse vs. no remorse) within-subject design. Forty-nine children aged 4 to 5 years (27 girls) were investigated by a classic paradigmof remorse and forgiveness. The “tear picture” game was used to create conditions of expressingremorse and not expressing remorse. Then, children were asked ten questions designed to assess whether they understood the true intention of remorse. Finally, the resource distribution task was used to assess forgiveness behavior. In Study 2, a total of 139 4- to 5-year-old children (80 girls) were recruited to participate in a 2 (expressions of remorse: remorse vs. no remorse) × 4 (types of bystander: no bystander vs. teacher vs. good friend vs. stranger) mixed experimental design. The procedure used in Study 2 was identical to that used in Study 1. The results showed that: In Study 1, both 4-year-old children and 5-year-old children trulyunderstood the intention of the expression of remorse, and they were much more forgiving of a remorseful transgressor than of a transgressor who had not shown remorse. In Study 2, althoughchildren were still more forgiving of a remorseful transgressor than an unremorseful transgressor in the presence of bystanders, bystanders significantly increased the level of forgiveness showntoward unremorseful transgressor and reduced the level of forgiveness for remorseful transgressor. Specifically, for remorseful transgressor, child victims were more forgiving of a transgressor while in the presence of strangers than while in the presence of teachers or good friends; however, for unremorseful transgressor, child victims who were being observed by teachers or good friends showed more forgiveness behavior than did victims who were being observed by stranger bystanders. Furthermore, the results showed that young children were more likely to equallydistribute flowers to remorseful and unremorseful transgressor in the presence of bystanders, especially teachers or good friends. This study successfully revealed that young Chinese children could accurately understand the intention of transgressors’ remorse and thus be willing to perform forgiveness behavior at age 4, which is exactly 1 year earlier than their counterparts in the Western sample. More importantly, we also present a new theoretical hypothesis, namely, the “bystander-activation effect of social expectations”, to propose that the presence of bystanders activates individuals’ socially desirable behaviors, such as “requite injury with kindness” and “egalitarianism”, in the Chinese collectivist culture. Thus, it is easier to understand why bystander onlooking could increase the level of forgiveness for unremorseful transgressor and cause young victims to distribute flowers toremorseful and unremorseful transgressor equally. Furthermore, the greater the authoritative or intimate level of bystanders is, such as teachers or good friends, the stronger the activated social expectations are and the more socially desirable the engaged-in behaviors are. This study provides important enlightenment for understanding the association between remorse and forgiveness andfor rethinking the cross-cultural differences in children’s socialization.

  • Impedance-based Stability Criterion of Oscillation Issues in Converter Caused by Phase-locked loop:Nominal Performance and Robustness

Subjects: Dynamic and Electric Engineering >> Electrical Engineering submitted time 2023-04-25

Abstract: There are many impedance-based methods to analyze the oscillation of grid-connected converter system (GCS) via dq frame, sequence frame and polar frame, which can be transformed into each other and are equivalent in judging the stability of the system. However, the discrepancy and adaptation of these methods has not been discussed theoretically. Therefore, the impedance models of GCS are established in above three kinds of coordinates respectively, then the mathematical and physical characteristics of them are analyzed. Secondly, the decoupling of impedance models in different frames are analyzed, and the differences of the impedance criteria in stability analysis and mechanism interpretation are given. Furthermore, the sub-synchronous oscillation caused by the phase-locked loop considering converter four quadrant operation scenario are illustrated, and the existence of open-loop transfer function right half plane poles and its sensitivity to uncertain parameters are analyzed from the nominal performance and robustness of the impedance criteria. On this basis, the adaptation of several impedance criteria of GCS are analyzed. Theoretical analysis and simulations confirm the validity of the proposed conclusion.

User rating data were collected for empirical analysis, and TOPSIS was used to calculate the comprehensive score of 30 knowledge-based microblogs, and the data was input into RBF neural network for simulation training. Result/Conclusion The test results show that the error between the simulation value and the actual score value of the knowledge-based microblog information quality evaluation model established in this paper is within 6%. The model can accurately fit the user scoring data, and is suitable for the microblog information quality evaluation scenario under the complex multi-dimensional index system, and has certain reference significance for the knowledge-based microblog and other types of microblog information quality evaluation.

  • The effects of mindfulness on eudaimonic well-being and its theoretical explanation

Subjects: Psychology >> Clinical and Counseling Psychology submitted time 2023-04-25

Abstract: Abstract: Eudaimonic well-being occurs when people’s life activities align with deeply held values, and they positively engage in activities to realize their potential. Eudaimonic well-being is characterized by a sense of purpose and meaning. Mindfulness is possibly the most centrally discussed intrapersonal factor influencing the pathway to enhance eudaimonic well-being. Recent research has found that mindfulness traits were particularly strong in relation to eudaimonic well-being. Attention awareness and acceptance can influence eudaimonic well-being by promoting positive qualities and experiencing fewer negative emotions in response to stress. Mindfulness-based training has been shown to improve eudaimonic well-being in clinical groups and non-clinical groups. S-ART model, self-determination theory, and mindfulness-to-meaning theory have provided a theoretical basis to explain the mechanisms underlying the effect of mindfulness on eudaimonic well-being from aspects of self-regulation of cognition, emotion, and behavior. Future research should focus on analyzing different effects of the basic components of mindfulness, exploring the mechanisms of mindfulness’s effects on eudaimonic well-being, and developing targeted mindfulness-based training programs to improve eudaimonic well-being.
