  • Development and Reliability and Validity Test of Cardiotoxicity Risk Assessment Scale for Breast Cancer Patients undergoing Chemotherapy

Subjects: Medicine, Pharmacy >> Clinical Medicine submitted time 2024-03-11 Cooperative journals: 《中国全科医学》

Abstract: Background Cardiotoxicity is a leading cause of mortality among breast cancer survivors and patients, making early risk assessment vital for the clinical prevention and treatment of cardiac toxicity. However, there is currently a lack of recognized tools for assessing the risk of cardiotoxicity in breast cancer chemotherapy patients in China. Objective To develop a cardiotoxic risk assessment scale for breast cancer patients undergoing chemotherapy and test its reliability and validity. Methods To develop a risk assessment scale for cardiotoxicity in breast cancer chemotherapy patients and to test its reliability and validity. Results Twenty medical staff experts were included in the semi-structured interviews. After two rounds of expert consultations,a 19-item risk assessment scale for cardiotoxicity in breast cancer chemotherapy patients was developed. In this study, there were 79 patients in the low-score group and 83 in the high-score group. Comparisons of endocrine treatment history and immunotherapy history showed no statistically significant differences between the two groups(P>0.05). Correlation analysis showed that smoking history and immunotherapy history were not significantly correlated with total scores(P>0.05). Eventually, three items-"smoking history,"endocrine treatment history" and "immunotherapy history"-were removed, resulting in a 16-item scale. The scale's Cronbach's alpha coefficient was 0.739; test-retest reliability was 0.983; inter-rater reliability was 0.984. The content validity results showed: I-CVI ranged from 0.83 to 1.00; S-CVI was 0.98. Predictive validity results showed: the scale's AUC was 0.887(95%CI=0.827-0.947,P<0.001),with a cut-off value of 32.50 points,Youden's index was 0.649,specificity was 89.1%,and sensitivity was 75.9%. Exploratory factor analysis results showed a KMO value of 0.700,and Bartlett's test of sphericity χ2value was 1037.898(df=120,P<0.001). The study extracted five common factors with a cumulative variance contribution rate of 61.991%. Risk stratification results indicated scores of 32-38 points as low risk,39-56 points as medium risk, and ≥ 57 points as high risk. Conclusion The cardiotoxicity risk assessment scale for breast cancer patients undergoing chemotherapy has good reliability and validity, which can better predict the high-risk population of cardiotoxicity and provide an effective assessment tool for clinicians and nurses to effectively identify the high-risk population of breast cancer patients undergoing chemotherapy.

Abstract: [Purpose/significance] Online health community has been developing rapidly, but it is difficult to reveal the inherent mechanism between network node attribute and user dynamic interaction mode, with the lack of research on node attribute-based dynamic networks. [Method/process] This paper constructed the research model for users temporal interactive mode of online health community by applying NATERGM (Node Attribute-based Temporal Exponential Random Graph Model), taking the online health community as research object, and combining with the feature of the user node properties that including users emotional tendency, the post text and the user community level. Then, data from Baidu Diabetes community from October 2016 to February 2018, which involves 2301 users, 6045 posts and 9490 replies, was used for empirical research. [Results/conclusion] Results indicate that the validity of NATERGM in analyzing temporal interactive mode, and node attributes have significant effects on reciprocity temporal mode, k-star temporal mode, transitivity temporal mode and cyclicity temporal mode. Consequently, suggestions for the development of online healthy community construction are put forward.

  • 内侧前额皮层-伏隔核环路在决策冲动中的作用:基于动物模型的研究

Abstract: Insufficient behavior control in patients with attention deficit / hyperactivity disorder (ADHD) is closely related to decision impulsivity, which is regulated by medial prefrontal cortex (mPFC) and nucleus accumbens (NAc). In ADHD patients, the mPFC and NAc show abnormality both structurally and functionally, indicating that these two brain regions are involved in regulation of decision-making, especially impulsivity. Although extensive anatomical connections between mPFC and NAc have been found, the role of mPFC-NAc circuit in decision impulsivity remains to be investigated. Wistar (WIS) rats and ADHD rats (SHR, spontaneously hypertensive rat) were used as subjects of this study. We recorded the local field potential (LFP) of mPFC and NAc using multi-channel electrophysiology during a delay discounting task (DDT). We further analyzed the coherence difference of Theta (4~12 Hz) oscillation in expectation period (0~3s) and compared this measure between the two groups. Results: (1) SHR rats had higher decision impulsivity level than WIS group. Power spectral density between 6~12 Hz of LFP in mPFC and NAc increased in both groups. (2) When choosing large/delayed rewards, coherence of mPFC-NAc activity increased compared to small/immediate rewards in WIS group. This indicates that the mPFC-NAc circuit mediates decision impulsivity. (3) When choosing large/delayed rewards, mPFC-NAc activity in SHR group showed lower coherence than WIS group, indicating SHR rats have weaker mPFC-NAc functional connections. (4) Coherence of mPFC-NAc activity is higher during initial choice behavioral than continuous choice behavior. It indicated that stronger mPFC-NAc functional connections are required during controlled information processing (dominant in initial choice behavioral), rather than automatic information processing (dominant in continuous choice behavior). Coherence of mPFC-NAc activity is higher in WIS group than SHR group when choosing large/immediate rewards. It indicated that the decision impulsivity deficits in SHR rats results from mPFC-NAc weak functional connections. (5) Coherence of mPFC-NAc activity is higher in shift trials than continuous trials. Plus, WIS group showed an overall higher coherence than SHR group. This indicated that mPFC-NAc circuit heavily involves in controlled information processing, and SHR group has deficiency of this process. (6) Regression analysis showed that coherence difference of mPFC-NAc activity in prediction period has positive correlation with delayed large reward selection rate in WIS group, that is, the more coherence of mPFC-NAc Theta activity increased during prediction period, the less decision impulsivity WIS rats behaved during choice period. However, the coherence difference cannot predict decision impulsivity in SHR group. Conclusion: mPFC-NAc heavily involves in decision impulsivity. Increase of coherence of mPFC-NAc theta oscillation in prediction period can predict impulsivity level. The study revealed that weak mPFC-NAc functional connections mediate decision impulsivity in ADHD and brought new perspectives in the treatment of ADHD.

  • EFFECT OF ELECTRON BEAM POWER ON TC4 ALLOY RIGID RESTRAINT THERMAL SELFCOMPRESSING BONDING, MICROSTRUCTURE AND MECHANICAL PROPERTIES OF JOINTS

Subjects: Materials Science >> Materials Science (General) submitted time 2023-03-19 Cooperative journals: 《金属学报》

Abstract: Rigid restraint thermal self-compressing bonding is a new solid-state bonding process. During the process, localized non-melted heating method is employed to heat the butted interface of the rigid restrained plates to be bonded. Under the localized heating, materials close to the butted interface are expanded. However, due to the existence of surrounding cool metals and rigid restraints, the expansion of the high temperature materials is restrained and thus, a compressive pressure is developed which compresses the high temperature metals near the bond interface and facilitates the atom diffusion between butt-weld specimens to produce a permanent solid-state joint. Utilizing the localized stress-strain field to accomplish atomic bonding, this process can avoid the use of external forces on which diffusion bonding and other solid-state bonding methods rely. Previous study has proven the feasibility of this process to join titanium alloys. In present work, the effect of beam power on bond interface, microstructure and mechanical properties of the TC4 joints bonded at different beam powers were analyzed through the OM observation, EBSD analysis, mechanical property test and fracture morphology analysis. Meanwhile, in order to reveal the mechanism about the effect of beam power on bond interface, the experiment study on microstructure and mechanical property and finite element analysis on present bonding were conducted to investigate the effect of beam power on the thermal stress-strain process during bonding. The results show that with the increase of beam power, the heating temperature, dwell time over high temperature, volume of materials with high temperature and the compressive plastic strain increase which promote the atom diffusion and thus bond quality of the interface is improved. At low beam power, the microstructure of the joints is homogeneous, while coarse grain with acicular a phase forms in the joint when the beam power is high. Mechanical properties of the joint are dependent on bond rate and microstructure. When the beam power is lower or higher, the compressive mechanical properties of the joints are poor because of the poor bonding quality of the interface or the coarse microstructure developed in the joint. Good comprehensive mechanical properties are obtained at the beam power of 330W.

  • The function of mPFC-NAc circuit in decision impulsivity- a study based on an animal model

Subjects: Psychology >> Physiological Psychology submitted time 2022-09-29

Abstract: Insufficient behavior control in patients with attention deficit / hyperactivity disorder (ADHD) is closely related to decision impulsivity, which is regulated by medial prefrontal cortex (mPFC) and nucleus accumbens (NAc). mPFC and NAc is involved in the regulation of decision-making process and impulsivity, and the structure and function of mPFC and NAc are abnormal in ADHD. The relationship between the functional coupling of mPFC-NAc circuit and abnormal decision impulsivity in ADHD is still under debate, abnormality of this circuit may provide explanations to the neural mechanism underlying ADHD.       Wistar (WIS) rats and ADHD rats (SHR, spontaneously hypertensive rat) were used as subjects of this study. We recorded the local field potential (LFP) of mPFC and NAc using multi-channel electrophysiology during a delay discounting task (DDT). We further analyzed the coherence difference of Theta (4~12Hz) oscillation in expectation period (0~3s) and compared this measure between the two groups.     Results: (1) SHR rats had higher decision impulsivity level than WIS group. Power spectral density between 7~9Hz of LFP in mPFC and NAc increased in both groups. (2) When choosing large/delayed rewards, coherence of mPFC-NAc activity increased compared to small/immediate rewards in WIS group. This indicates the mPFC-NAc circuit involves in decision impulsivity. (3) When choosing large/delayed rewards, mPFC-NAc activity in SHR group showed lower coherence than WIS group, indicating SHR rats have weaker mPFC-NAc functional connections. (4) Coherence of mPFC-NAc activity is higher during initial choice behavioral than continuous choice behavior. It indicated that stronger mPFC-NAc functional connections are required during controlled information processing which is dominant in initial choice behavioral, while automatic processing is dominant in continuous choice behavior. Coherence of mPFC-NAc activity is higher in WIS group than WIS group when choosing large/immediate rewards. Decision impulsivity in ADHD as a consequence of the dysfunction, is caused by a weak mPFC-NAc functional connection.
