  • Emergency nursing experience of intravenous thrombolysis in a patient with acute cerebral infarction

    Abstract: Acute cerebral infarction is one of the common diseases in emergency medicine, and it is a disease with a treatment "time window", with a high disability rate. Early opening of the "stroke green channel" and early implementation of "intravenous thrombolysis" treatment are key to reducing disability and ensuring the prognosis and quality of life of patients. This article reviews the thrombolysis nursing process of a patient with acute cerebral infarction and summarizes the experience of standardized thrombolysis nursing for stroke.
     

  • Correlation between Subclinical Left Ventricular Systolic Function and Diabetic Microvascular Complications in Patients with Type 2 Diabetes

    Abstract: Background  In recent years,studies have demonstrated the use of ultrasound speckle tracking techniques for the early evaluation of subclinical left ventricular(LV) systolic function in patients with type 2 diabetes mellitus(T2DM). In addition,long-term exposure to hyperglycemia severely damages the microvascular system. However,the interaction between diabetic microvascular complications and subclinical LV systolic function has not been fully understood. Objective  To investigate the relationship between subclinical LV systolic function and diabetic microvascular complications in patients with T2DM. Methods  A total of 150 patients with T2DM who were admitted to the Department of Endocrinology,the First Affiliated Hospital of Air Force Medical University from June to December 2021 were selected in the cross-sectional study and all underwent conventional echocardiography,pulsed tissue Doppler echocardiography and two-dimensional speckle tracking echocardiography(2D STE). The 2D global longitudinal strain(GLS) of LV was obtained to assess the subclinical LV systolic function. Diabetic microvascular complications were defined as combined diabetic retinopathy,diabetic kidney disease or peripheral neuropathy. The subjects were divided into the T2DM alone group(T2DM alone group,n=76),the T2DM with 1 complication group〔T2DM+C(1) group,n=37〕 and the T2DM with 2 to 3 complications group〔T2DM+C(2 to 3)group,n=37〕 according to the cumulative number of microvascular complications. The general demographic characteristics and biochemical indices of patients in the three groups were compared. The relationship between microvascular complications and subclinical LV systolic function was assessed by using Spearman correlation analysis and Logistic regression analysis. Results  GLS was lower in the T2DM+C(1) and T2DM+C(2 to 3) groups than in the T2DM alone group (P<0.05). Spearman rank correlation analysis showed that the number of diabetic microvascular complications involved was negatively correlated with GLS(rs=-0.349,P<0.001). Logistic regression analysis showed that after adjustment for multiple factors of gender,age,duration of diabetes,hypertension,glycosylated hemoglobin,fasting glucose,blood creatinine,heart rate,urinary microalbumin,and LV fraction shortening,GLS was decreased in the T2DM+C(1) group〔OR=0.744,95%CI(0.601,0.920)〕 and the T2DM+C(2 to 3) group〔OR=0.707,95%CI(0.525,0.952)〕 compared with the T2DM alone group (P<0.05). Multivariable Logistic regression analysis based on hypertension stratification showed that after adjustment for the same confounding factors described above,diabetic microvascular complications remained independently associated with reduced GLS〔with hypertension:OR=0.556,95%CI(0.321,0.963);without hypertension:OR=0.739,95%CI(0.558,0.979),P<0.05〕 in T2DM patients with or without hypertension. Conclusion  The LV systolic function may already be impaired in hospitalized T2DM patients with diabetic microangiopathy. The increased number of microvascular complications involved was closely related to subclinical LV systolic dysfunction,independently of hypertension.

  • Evaluation of Medical Level in China by Provinces Based on Principal Component Analysis and TOPSIS Model

    Abstract: Background During the nationwide epidemic of COVID-19 infection,the spatial agglomeration of medical resources in China has been highlighted,and there are obvious differences in medical level among provinces. Currently,the evaluation of medical level in China by provinces was mainly conducted by quantitative methods by domestic scholars,while comprehensive method was less applied to evaluate the medical level by provinces. Objective To understand the differences in the level of healthcare development in China by provinces,so as to provide a reference for healthcare decision makers. Methods In November 2022,CNKI,Wanfang Data Knowledge Service Platform,and Web of Science were searched by computer for the researches in the field of medical level. Based on the existing research results,relative and average indicators were selected to construct the evaluation index system. The data of each evaluation index was extracted or calculated by using China Health and Health Statistical Yearbook 2022 as the data source. Using the principal component analysis and TOPSIS model,the medical levels of 31 provinces in China(Hong Kong Special Administrative Region,Macao Special Administrative Region and Taiwan Province were not included in the statistics)were comprehensively evaluated. Results A total of 6 qualified papers were retrieved and 13 relative and average indicators were selected from three aspects of medical resources,medical services,and medical security to construct the evaluation system. The KMO value was 0.733,and Bartlett's spherical test showed that χ2=346.908,P<0.001,suggesting that the data were suitable for principal component analysis;four principal components were extracted according to the criterion of characteristic root above 1.000,including the scale of medical resources and quality of medical services(F1),the efficiency of medical institutions(F2),infectious disease control ability(F3),and other factors(F4),and the cumulative percent variance of the four principal components was 84.012%. After establishing the linear model of each principal component based on the matrix of the principal component scores,the comprehensive evaluation model for the medical level was obtained based on the cumulative percent variance of the four principal components:Y=0.439 85×Y1+0.158 54×Y2+0.154 40×Y3+0.087 34×Y4. The top three provinces in terms of comprehensive score of medical level were Beijing(151.908 points),Shanghai(124.379 points),and Tianjin(78.673 points). The TOPSIS proximity ranking showed that Beijing and Shanghai were at the top level(proximity was 0.767 and 0.646,respectively),and the 31 provinces could be divided into three echelons with proximity 0.400 and 0.200 as the nodes. The first echelon included three provinces of Beijing,Shanghai and Tianjin,the second echelon included 25 provinces such as Zhejiang Province and Sichuan Province,the third echelon included three provinces of Hebei Province,Ningxia Hui Autonomous Region and Tibet Autonomous Region. Conclusion There is an obvious imbalance in the level of medical development in China by provinces,showing an olive#2;shaped structure of "big in the middle and small at the two ends" in the overall distribution of medical level in 31 provinces. The government should increase the incline degree of policy for provinces with low ranking in medical level,such as Hebei Province,play a coordinating role in regional health planning,and implement targeted assistance by using telemedicine and medical big data.

    Abstract: The essence of public administration is public decision-making. Social license of public decision (SLPD) refers to the extent to which local people accept and support a public decision from government or public authorities. Lack of this license not only hinders the efficiency of government policy, but also affects the decision-making authority and long-term goals of society. Moreover, government transparency is regarded as an important factor to eliminate public decision-making dilemma and enhance administrative legitimacy both in value and democratic practices. This research explored the causal relationship between transparency of government decision-making (i.e., transparency in process and transparency in rational) and SLPD from the perspective of Behavioral Public Administration (BPA), which is a bridge linking Public Administration and Psychology. In other words, BPA is a new interdisciplinary sub-field of Public Administration from Psychology. The research of BPA mainly focuses on the process between government decision-making and citizen experience. In addition, based on the concept of bounded rationality and heuristic judgment as well as system justification theory, we built and tested the moderating roles of trust in government and outcome dependence between transparency of government decision-making and SLPD in two models. Outcome dependence is the extent to which someone is dependent on a powerful authority (i.e., the representative of a system) when that authority controls valued resources whose social and/or material outcome the person desires. This research includes three studies, two survey experiments (N = 354 + 354) and one field survey (N = 520). The studies were conducted in China. The results showed that: First, transparency of governmental decision-making positively influenced SLPD. That is, for both transparency in process and transparency in rationale of government decision-making, the higher the transparency is, the higher the SLPD is. Second, trust in government moderated the relationship between transparency in rational and SLPD. Specifically, the positive relationship between transparency in rational and SLPD gets weakened when the trust in government is higher. Third, outcome dependence moderated the relationship between transparency of governmental decision-making and SLPD. Specifically, the positive relationship between two types of transparency and SLPD gets weakened when the outcome dependence is higher. Therefore, “Transparency effects” of SLPD was proposed through the present research. In addition, “cautious indifference” was used to indicate the moderating role of trust in government, and “selective neglect” was used to indicate the moderating effect of outcome dependence. The theoretical contributions were embodied in three aspects: (1) defining a new concept (i.e., SLPD); (2) introducing a new perspective (i.e., BPA); (3) discovering a new mechanism (i.e., transparency effect and its moderators). Regarding the practical implications, this research could shed light on the transparency practice, and provides empirical evidence to government for further enhancing the legitimacy of public decisions.

    Abstract: In order to further strengthen the standardization and homogenization of nursing care for severe and critical patients and give full play to the advantages of integrated traditional Chinese and Western medicine nursing, Beijing Administration of Traditional Chinese Medicine (TCM)commissioned the Beijing Office of TCM Nursing Capacity Improvement Project to organize the specialized group to compile Nursing Specification of Integrated Traditional Chinese and Western Medicine for severe and critical patients with COVID-19.This specification elaborates the nursing contents of integrated traditional Chinese and Western Medicine for severe and critical patients with COVID-19 in detail,including etiology and pathogenesis,severe/critical high-risk groups,clinical classification and nursing measures,etc.Among them,nursing measures include disease observation and nursing,medication nursing,respiratory support care,prone position treatment and nursing,nutritional support nursing,general nursing,convalescent nursing,etc.This specification is convenient for clinical nurses in various medical institutions to use as reference,and provides reference for clinical application of integrated Chinese and western medicine in nursing severe and critical patients infected with COVID-19.
     

  • Hot Topics and Evolution Research of Archives in China Based on LDA Model

    Abstract:

    [Purpose/significance] The LDA model is used to discover the research hotspots and development trends of Chinese archival science in recent years, which provides data support and reference value for the research of Chinese archival science. [Method/process]Select the Chinese abstracts of 9 core journals of archival science from 2012 to 2022 as the analysis sample, take the CNKI database as the source database, use the Python open source toolkit pkuseg to carry out Chinese word segmentation, build the LDA model with gensim, and use pyLDAvis presents interactive visualization of various topics based on web. According to the visualization results of pyLDAvis, the topic is named. According to the probability distribution of document-topic and the time item, the hot topics and the topic evolution process are analyzed. [Result/conclusion]According to the LDA model, it is possible to effectively distinguish the topics of research in the field of archival science in China. From 2012 to 2022, there are 14 topics in the field of archival science in China, including 5 hot topics; Three topics showed an upward trend, one topic showed a downward trend, and 10 topics showed a band trend of varying degrees.

  • Reasons and countermeasures of nursing disputes in the nucleic acid testing clinic

    Abstract: Objective To investigate the factors of high incidence of nursing disputes in the nucleic acid testing clinic, and to optimize management and reduce the incidence of nursing disputes. Methods A retrospective before-after study was conducted. Potential reasons of nursing disputes happened between April and September 2021 were retrospectively analyzed, and countermeasures were carried out to optimize management in the nucleic acid testing clinic. The incidence of nursing disputes happened between October 2021 and January 2022 was calculated. Results Issues related to patients, nurses and working environment were main factors causing nursing disputes. The incidence of nursing disputes had decreased from2.35? to 1.56? after implementation of optimal management strategy. Conclusion In order to reduce the incidence of nursing disputes and improve the nursing quality in the nucleic acid testing clinic, it is required to enhance the nursing staff’s sense of responsibility and effective communication with patients, standardize the nursing operation process, strengthen cooperation between various departments and improve the hospital workplace environment.

  • Observation on the efficacy of MAO Stain technique in treating acute herpes zoster

    Abstract:

    Objective To investigate the effect of MAO sweat treating herpes zoster.Methods: A total of 60 patients with herpes zoster treated in our department from May 2021 to January 2022 were randomly divided into observation group and control group, with 30 cases in each group. The control group was treated with conventional western medicine, and the observation group was treated with MAO stain of Traditional Chinese medicine. The pain and efficacy of the two groups were compared.Results: After intervention, the pain and curative effect of herpes zoster patients in the two groups were improved, and the improvement of the observation group was better than the control group. Conclusion: Good sweat can improve the curative effect of herpes zoster.

  • Application of cold compress of Traditional Chinese Medicine in a patient with wind-warm and pulmonary heat syndrome

    Abstract:

    This paper summarized the application effect of cold compress of Traditional Chinese Medicine (TCM) in a patient with wind-warm and pulmonary heat syndrome. According to the outcomes of nursing evaluation and diagnosis, the TCM cold compress therapy was carried out in combination with nursing interventions such as daily care, nutrition guidance, emotion nursing, and so on. The TCM cold compress therapy has obvious effect on body cooling during fever caused by wind-warm and pulmonary heat syndrome, and it demonstrates the characteristics of easy-to-use, convenience and cost-effectiveness.

  • 基于流式细胞术的乌饭树核DNA含量(2C-值)测定

    Abstract:核DNA含量(2C-值)是描述植物生物多样性的一个重要特征参数。本试验利用流式细胞仪检测越橘属植物乌饭树核DNA含量。建立了适合乌饭树基因组的流式细胞术测定方法:以野生乌饭树的嫩叶为材料,以已知核DNA含量的水稻品种‘日本晴’为内标,采用GPB解离液,细胞核悬液加入50 μL∙mL-1碘化丙啶染色5 min即可上机检测。9个乌饭树单株的核DNA含量平均值为1.22±0.03 pg,最小值为1.18 pg,最大值为1.27 pg。检测结果与已知的越橘属二倍体植株的2C-值含量相似,且不同地理来源的单株DNA含量没有显著差异(P>0.05),推测这9个单株为二倍体植株。本研究测定的乌饭树核DNA含量(2C-值)可丰富越橘属植物的C-值库;基于流式细胞术建立的乌饭树核DNA含量测定方法可为该属其他植物的相关研究提供借鉴。
