  • Effect of laser lithotripsy of giant bile duct stones under endoscopic retrograde cholangiopancreatography and nursing management

    Abstract: Objective To compare the efficacy of dual-frequency dual-pulse laser lithotripsy under endoscopic retrograde cholangiopancreatography (ERCP) and traditional endoscopic mechanical lithotripsy in the treatment of patients with common bile duct stones, and summarize perioperative nursing measures. Methods From May 2018 to May 2022, the clinical data of 200 patients with common bile duct stones who underwent ERCP treatment and lithotripsy were retrospectively analyzed, of which 69 patients were treated with dual-frequency dual-pulse laser lithotripsy (Laser group) and 131 patients were treated with mechanical lithotripsy (Mechanical group), and the success rate of stone extraction, operation time, postoperative hospital stay time and complications of two groups of patients were compared. Results There was no significant difference in baseline data such as general conditions and preoperative clinical data between two groups (P>0.05). Neither neither Laser group nor Mechanical group had reported the perioperative death case. There were no significant difference in postoperative bleeding, postoperative pancreatitis and perforation complication rates in the two groups (P>0.05). The surgical time in the Laser group was significantly longer than that in the Mechanical group (P<0.05). The postoperative hospital stay was shorter in the Laser group than that in the Mechanical group(P<0.05), and the total complication rate and stone residue rate were lower in the Laser group than those in the Mechanical group (P<0.05). Conclusion The treatment of common bile duct stones by dual-frequency dual-pulse laser lithotripsy has better efficacy and lower complication rate than traditional surgical mechanical lithotripsy, but there is still room for improvement in the operation time.

  • Research on the role of assisted compressions of the abdomen in colonoscopy

    Abstract:

    Objective To investigate the safety and efficacy of assisted abdominal compression during colonoscopy. Methods A total of 50 patients who came to the endoscopy center of our hospital for colonoscopy were randomly divided into 25 patients in the personnel-assisted turning without compression group (group R) and 25 cases in the abdominal group (group D) with assisted compression, and the number of abdominal discomforts and colonoscopy time during the examination of the two groups of patients were recorded in the examination procedure. The effectiveness of abdominal compressions was compared between the two groups of patients. Results There were 25 cases in the assisted turnover group without compressions (group R) and the abdominal group with assisted compressions (group D), and there was a statistically significant difference in the number of abdominal discomforts in the examination procedures (P<0.05). The difference in colonoscopy time was statistically significant (P<0.05). Conclusion The application of assisted compression of the abdomen is to further reduce the pain of colonoscopy patients and the workload of the operator in the clinic, which has a certain effect.

  • Nursing study of cold resection of colon polypecy snare and endoscopic mucosal resection

    Abstract:

    Objective To analyze the effect of endoscopic mucosal resection (EMR) and cold snare polypectomy (CSP) in the treatment of colon polyps. Methods A total of 200 patients with colon polyps admitted to the Oriental Hospital of Beijing University of Chinese Medicine from January 2020 to December 2020 were selected as the research objects, and 100 cases (154 polyps) were divided into EMR group (172 polyps) and 100 cases (154 polyps) in the CSP group according to the random number table method. The two groups of polyp resection time, complication rate, polyp complete resection rate, specimen recovery rate, polyp recurrence rate and polyp regeneration rate were compared and analyzed. Results 154 polyps were removed in the CSP group and 172 polyps in the EMR group, and there was no statistical significance in the comparison of the complete resection rate and specimen recovery rate in the two groups (P>0.05). The polyp excision time in the CSP group was shorter than that in the EMR group (P<0.05), and the complication rate difference between the two groups was not statistically significant (P>0.05). Six months after surgery, there was no significant difference in the incidence of polyp recurrence and polyp regeneration in the two groups (P>0.05). Conclusion CSP treatment of colon polyps can shorten the operation time, and the complete resection rate, recurrence rate, polyp regeneration rate and safety of polyps are similar to EMR.

  • 泛基因组研究在遗传多样性和功能基因组学中的应用

    Abstract: The pan-genome can represent all of the genetic diversities in a species or population, which is a limitation for obtaining only one single reference genome. The pan-genomics is becoming a new hot research area and being widely applicated in researches of many species in plants, animals and microorganisms, as the development of the whole genome sequencing and analysis technology. It provided powerful tools for resolving the genetic variation and polymorphism at levels of species or taxa, researches of functional genomics and reconstruction of phylogenetics, obtaining abundant of significant research achievements. However, present researches on pan-genomics still need improve due to several problems, e.g., extensive cost of whole genome sequencing and data analysis, inconsistent analysis standards, lack of deeper and comprehensive explanation of the obtained data, and difficulty of application of the research achievements. We summarized the research progresses of pan-genomes on exploitation of genetic diversity and functional genomics, including construction of a pan genome map, identification of genome variations and favorable genes, polymorphism of functional genes, population genetic diversity and systematic evolution, and discussed its potential in application of different research fields. Furthermore, we discussed the limitations existed in the present studies and possible solutions, and directed the development prospect in the future on pan-genomics.
