Effect of Periodic Leg Movements in Sleep on sleep structure in stroke patients
摘要:背景 卒中后患者可伴有不同类型的睡眠障碍,临床多见却易忽视周期性肢体运动(PLMS)对睡眠的影 响,国内外报道PLMS 对卒中患者的影响较少,需进一步研究。目的 探讨周期性肢体运动对脑卒中患者的睡眠影 响。方法 选取2020 年12 月-2022 年2 月于我院就诊,伴有睡眠障碍且行多导睡眠监测脑卒中患者81 例。根据睡 眠周期性肢体运动指数(PLMI)15 次/h 为界分为:PLMS+组(PLMI15)、PLMS-组(PLMI15)。比较并分析两组 之间的睡眠结构。结果 两组比较,PLMS+患者睡眠效率降低、觉醒指数增高,二期睡眠比例增加、三期睡眠比例减 少、清醒时肢体运动指数增加,差异具有统计学意义(P0.05);余睡眠分期及睡眠呼吸暂停低通气指数(AHI)差 异无统计学意义(P0.05)。结论 研究表明伴有觉醒的周期性肢体运动的脑卒中患者,客观存在睡眠紊乱,为心 脑血管疾病中的重要危险因素。
英文摘要:Background Post-stroke patients may have different types of sleep disorders, but the effect of periodic limb movement (PLMS) on sleep is often overlooked in clinical practice. There are few reports about it, which needs further study. Background As a common complication of stroke patients, sleep disorders are clinically common but easy to ignore the impact of PLMS on sleep.There are few reports about it at home and abroad, which need further discussion. Objective The current study was designed to investigate the effect of sleep periodic limb movement(PLMS) on sleep of stroke patients. Methods From December 2020 to February 2022, 81 stroke patients with sleep disorders and polysomnography were selected from Department of Neurological Rehabilitation,the Second Affiliated Hospital of Zhengzhou University. Accordingto to the periodic limb movement index(PLMI),those patients were divided into PLMS group (PLMI>15times/hour,PLMS+)and non-PLMS group (PLMI compared between both groups. Results The sleep efficiency and the proportions of Ⅲ stage of non-rapid eye movement sleep(NREM-Ⅲ)of PLMS group were lower than non-PLMS group. The arousal index,the proportions of Ⅱ stage of non-rapid eye movement sleep(NREM-Ⅱ),Conscious limb movements of PLMS group were higher than non-PLMS group. They were significant as being compared between the two groups(Pvalue hypopnea index(AHI) were not significantly different (Pvalue >0.05). Conclusion The study shows that stroke patients with arousal periodic limb movement have sleep disorders, is one of most important risk factors for cardiovascular and cerebrovascular diseases.
